PHARMACY RETURN KIT

Includes printable instructions for pharmacies on how to prepare a return along with an inventory sheet to document the items being sent in, as well as a Schedule-II return request form for any Schedule-II pharmaceuticals.

ONLINE C-II REQUEST

All Schedule-II drugs require a DEA 222 Form prior to being sent back to PharmaLink. A DEA 222 Form request may be submitted via fax, mail or, alternatively can be filled out completely online. Upon receiving your request, our Compliance Department will promptly send out a DEA 222 Form. Please remember to follow the instructions carefully when you submit an online request; failure to follow the instructions could result in a delay with processing your request.
